This study assessed whether the strength of an exacerbating stimulus might explain the consistent dichotomy in duration of fatal asthma attacks. Although total mast cell numbers were similar, the ratio of degranulated to intact mast cells and proportional muscle shortening were increased in short-duration compared with long-duration cases of fatal asthma. This is consistent with a stronger exacerbating stimulus.
