N2 - The Collaborative Care Cluster of Australia (CCCA) project`s aim was to roll out care models and e-Health solutions for patients with chronic disease. This includes improved quality and safety of care, increased efficiency and more accessible care. The two year project was principally located in the Melbourne metropolitan area. Led by Precedence Health Care (PHC), the 12 consortium members represent key stakeholder constituencies in collaborative care. They comprise Fred Health, Cisco, Monash University, CSIRO e-Health Research Centre, Baker IDI, Southern Health, Royal District Nursing Service (RDNS), Dandenong-Casey GP Association, Bupa, Diabetes Australia Victoria, The Royal Australian College of General Practitioners (RACGP) and the Department of Health Victoria (formerly the Department of Human Services).
