Ovarian Endometrioma: Surgery and Fertility Preservation

Abstract

Ovarian endometrioma or "chocolate cyst" is a type of deep endometriosis. It is rarely a solitary lesion: the vast majority of women diagnosed with an endometrioma will have other lesions, often adding up to severe disease classifications. It is no surprise, then, that these lesions are often associated with infertility. The management of endometriomas in the context of infertility has recently been the subject of much debate, especially since the growing success of in vitro fertilization. The two questions at the center of the debate are whether surgery still has a place and if so, which procedure gives the best results. This chapter will attempt to answer these questions with the best currently available evidence.
