Occupational outcomes for Parkinson’s disease from the Lee Silverman Voice Treatment program

Abstract

Introduction: Parkinson’s disease (PD), a progressive neurodegenerative disorder, impacts motor and nonmotor body systems. Lee Silverman Voice Treatment (LSVT) BIG® is a movement-based program that utilises repetitive, high-intensity exercises to recalibrate movement, improve balance, and reduce bradykinesia for people with PD. Existing research has examined the effectiveness of LSVT-BIG® on non-motor symptoms including cognition, mood, quality of life, and occupational performance.
Objective: This study aimed to explore the effectiveness of LSVT-BIG® on non-motor and occupational outcomes for people with PD.
Method: A mixed methods retrospective record review was conducted with 69 participants, completing LSVTBIG ® at a Melbourne private hospital between 2015 and 2021. Outcome measures included Geriatric Depression Scale (GDS), Montreal Cognitive Assessment (MoCA), Parkinson’s Disability Questionnaire (PDQ-39), and the Canadian Occupational Performance Measure (COPM). Scores from pre–post and 3-month post-LSVT BIG were analysed using the Wilcoxon signed-rank test to determine change over time. Participant COPM goals were categorised into self-care, leisure, and productivity using content analysis.
Results: Significant differences in scores pre- and post-LSVT-BIG® were found for MoCA (Z = 2.879, P = 0.004), GDS (Z = 2.614, P = 0.009), PDQ-39 (Z = 3.258, P = 0.001), COPM Performance (Z = 5.580, P = 0.001), and COPM Satisfaction (Z = 5.036, P = 0.001). The most common COPM goals were self-care, predominantly, functional mobility and personal care.
Conclusion: These findings suggest LSVT-BIG® is a beneficial intervention to improve non-motor and occupational performance outcomes associated with PD and suggests clients’ perspectives can be valuable when evaluating the effectiveness of intervention programs.
