Abstract
Transitions of care remain a significant source of medication-related harm. While a ‘transition of care’ encompasses any change in setting or healthcare provider, medication-related harm is often most prevalent after admission to or discharge from hospital. This is due to the complex interaction of people, technology, systems and policies that occur between hospital and primary care or residential aged care. With the World Health Organization (WHO) Global Patient Safety Challenge: Medication Without Harm, aiming to reduce medication-related harm by 50%, it is not surprising that transitions of care has been identified as one of this challenge’s three key priority areas.
