Abstract
We read with interest the systematic review and meta-ethnographic synthesis by Kandiahet al. which provides useful insights into approaches to improve medication safety during transitions of care from hospitals to care homes.
The systematic review excluded studies that evaluated interventions delivered outside of care homes. As hospitals are a key component of the hospital-to-care home transition, interventions in the hospital setting need to be considered within integrated care design. Exclusion of hospital-delivered interventions may have led to relevant insights and contextual factors being overlooked in the meta-ethnographic synthesis.
