Empathy in the digital age: Using AI simulations for enhancing cultural competence in healthcare education

Abstract

In today’s increasingly diverse and interconnected world, there is a growing imperative for cultural competence in healthcare to ensure equitable and effective patient care across varying cultural contexts. This project presents findings from a recent pilot study featuring “Aalia”, an AI-driven simulated patient, created to immerse health professions students in a complex cultural scenario. Aalia is a 32-year old Middle Eastern woman navigating the Italian healthcare system, and her journey challenges students to engage deeply with her cultural and religious needs. This immersive educational experience bridges the gap between technical skills and empathetic care, encouraging students to develop a more culturally sensitive approach to patient centred care. Leveraging advanced AI technology, ‘Authentic Teaching and Learning through Adaptive Simulations’ (ATLAS) software within a ‘Collaborative Online International Learning’ (COIL) framework, students across ten health professions disciplines and four countries interacted with Aalia’s case. Through structured interactions with Aalia, students were encouraged to engage with her experience empathetically, learning to identify and address cultural factors that influence healthcare dynamics. Initial findings from the project, involving over 130 students and 20 educators, indicate substantial improvements in students’ cultural awareness, bias recognition, and clinical decision-making. This digital interaction enabled students to form a meaningful, empathetic connection with Aalia, thereby enhancing their capacity for culturally responsive care in diverse healthcare environments. Through a systematic analysis of student reflections and clinical interactions with this AI persona Aalia, this project highlights the effectiveness of simulation-based education and digital educational technologies in fostering empathy and promoting peer-to-peer learning within intercultural and interprofessional educational programs. Our findings suggest that immersive, AI-driven tools can bridge gaps in traditional health professions education by providing an emotionally resonant, patient-centred learning experience, equipping students to deliver equitable, patient-centred care in multicultural settings.
