This study will evaluate how accurately high frequency activity (HFA) identifies brain areas critical for language. We will compare high frequency activation sites to those identified by direct cortical stimulation, the current gold standard, and examine whether removing HFA positive areas during surgery is linked to language decline after the epilepsy surgery.
Significance:
HFA mapping could improve the safety and accessibility of epilepsy surgery by reducing the need for electrical stimulation and enabling language localisation even when stimulation is not possible. This study builds on promising pilot data and aims to develop a predictive framework—using HFA signals to identify critical language areas and forecast language outcomes after surgery—supporting broader clinical use.
