Breakdowns in communication contribute enormously to adverse events that lead to serious harm. This study will examine health professionals' communication in how they manage medications as consumers move across transition points from the emergency department to other environments. Working with The Alfred, the study will lead to new knowledge of the complexities involving communication from the perspectives of health professionals, consumers and carers across the continuum of care, and of how health professionals address gaps in communication. The outcome of the study will be the development of strategies for improved communication in how medications are managed across various environments, with a focus on reducing medication adverse events.
