Project Description

The Tasmania Community of Practice StrokeLink Program is a two-year project funded by the Crown through the Department of Health and Human Services in Tasmania. The Program aims to use performance monitoring data to underpin quality improvement activities, including the externally facilitated StrokeLink program, in consultation with clinicians and other stakeholders. The approach will be consistent with a national framework for collection of data that enhances awareness of gaps in care to inform quality improvement efforts.
The program aims to establish a Tasmanian Community of Practice for stroke providing clinical leadership to improve access to best practice stroke treatment and care across the state (StrokeLink). As part of this program Monash has agreed to provide input and support to some of the strategies in the program.
