Project Description

This innovative project takes a whole-of-system approach to identify and target serious medication safety issues which are most likely to cause preventable hospitalisations, morbidity and mortality. The intervention will be undertaken at the primary care level, initiated by pharmacists and actioned within an integrated primary care framework. Using a pre-specified list of clinical indicators, at-risk individuals will be identified through existing electronic clinical records designed to be compatible with My Health Record in the future for nationally scalability. A co-design process will be used to incorporate the preferences of consumers, particularly vulnerable groups such as Aboriginal and Torres Strait Islander peoples.
